What is Fibromyalgia?
Fibromyalgia is a chronic disorder characterised by widespread musculoskeletal pain, fatigue, sleep disturbances, cognitive problems (such as memory and concentration issues), and often co-existing conditions like irritable bowel syndrome (IBS) and migraines. It is a complex condition with unknown causes, but it is believed to involve a combination of genetic, environmental, and psychological factors. Fibromyalgia affects mainly women and can significantly impact a person’s quality of life, making daily activities challenging. While there is no known cure for Fibromyalgia, various treatment approaches, including medication, lifestyle changes, physical therapy, and stress management techniques, can help manage the symptoms and improve overall well-being. Individuals with Fibromyalgia need to work with healthcare professionals to develop a personalised treatment plan.
What is Chronic pain?
Chronic pain is a persistent pain that lasts longer than 12 weeks despite medication. This pain can happen after an injury or surgical operation, but in the same cases, the pain comes on without any injury history.
What is pain?
Pain is an unpleasant feeling, such as a burn, stinging, or tingling sensation, which could be sharp or dull, depending on the magnitude of the damaged tissue involved. The nervous system signals the brain that something is wrong. The brain and the nerves inside the spine (spinal nerves) form the central nervous system. The spinal nerves transmit signals from the physical body to the brain, the brain acts like a control panel, and the brain interprets this information. In some cases, the interpretation is inaccurate. We often expect that the pain will subside with time; however, the brain keeps sending these pain signals and the pain increase at times for no reason.

Chronic fatigue syndrome (CFS)/myalgic encephalomyelitis (ME)
Chronic fatigue syndrome (CFS) is known as myalgic encephalomyelitis (ME). It is often related to immune system abnormalities. It can present with extreme tiredness or fatigue that does not resolve with simple rest and is not associated with other medical conditions.
Patients often report sleep disturbances such as insomnia and constant exhaustion, even upon waking up in the morning. These patients started losing brain function gradually with poor memory, low concentration, frequent sore throat, headaches, in some cases, postural faintness; the underlying cause of this devastating syndrome is not apparent; however, it is frequently related to viral infection or psychological stress. The diagnosis is made by exclusion; this means that other causes of fatigue are being ruled out.

If you have persistent excessive and undiagnosed tiredness, see your GP. Constant tiredness can be a symptom of many illnesses, such as infections or other physiological disorders.

Conventional Treatment
Pain, headaches, and sleep disturbance treatments can include antidepressant therapy or referral to a mental health clinic.
Fibromyalgia patients are generally referred to a hospital to see a rheumatologist, neurologist or psychiatrist. The treatment focuses on managing the symptoms with a combination of medications, such as painkillers or other more potent drugs such as codeine and tramadol, antidepressants such as venlafaxine, paroxetine and fluoxetine, muscle relaxants such as diazepam, and anticonvulsants medications such as gabapentin.
Our Integrative Functional Approach
Chronic fatigue and pain are common symptoms of fibromyalgia and CFS; the main difference is that CFS patients have an overwhelming lack of energy; however, these patients can also experience some pain. In fibromyalgia patients, fatigue usually takes a backseat to debilitating muscle pain, presenting tenderness and pain all over their bodies. These patients become extremely tired and have sensitivity to pain, muscle stiffness, sleep disturbances, brain fog and difficulty with concentration and memory.
In some cases, the reason can include gluten sensitivity, candida overgrowth or mercury toxicity. Some patients may have an underactive thyroid, adrenal fatigue, nutritional deficiencies or genetic mutations such as methylenetetrahydrofolate reductase (MTHFR) or absent glutathione S-transferase M1 (GSTM1) or glutathione S-transferase P1 (GSTP1).
MTHFR – causes an increase in homocysteine levels and other conditions such as high blood pressure, blood clots and certain types of cancer
GSTM1 – which plays a crucial role in the detoxification of metabolites of environmental carcinogens
GSTP1 – which can increase the susceptibility and risk to hepatocellular carcinoma.
Fibromyalgia and ME have an almost similar approach. The first line of advice is to adopt an anti-inflammatory diet of whole-natural food or fruits and vegetables. Eliminate allergens such as gluten and dairy products, and check for hiding infections such as bacterial, viral or yeast.
The management of chronic fatigue focuses on restoring mitochondrial function. These are intercellular energy vulnerable to damage by free radicals, and CFS presents with fatigue, memory loss, pain and accelerated ageing.
